Update Information - Name Change - General

Please complete the Employee Change Request Form and provide the requested documents.

It is also recommended that you visit your LAPP Pension Profile to update any personal information, as well as Canada Life if you have additional life insurance or a Group Savings Plan.

Once the Employee Change Request Form is completed, it will automatically be directed to Employee Services for processing.
